How to submit to Healthgrades Provider Profile

Steps:
1. Open the claim page and search by location and the provider's name.
2. Enter the DEA or state license number when asked, which Healthgrades uses to verify identity.
3. If the profile is missing, try the NPI-based search or contact Healthgrades.
4. Create the account, then update specialties, office details and accepted insurance.

Watch out for:
- Profiles are created from public data, so this is a claim flow rather than a submission form.
- No outbound website link could be confirmed on a free profile, so the link type is unknown.
- Some of Healthgrades' directory pages return 410, so expect old URLs to fail.
